Cannot open (Database does not exist or access denied)

EBM uses a Microsoft SQL database service, which runs on the local computer (stand-alone installations), or on a network server (network installations).  If this service is not running, EBM will not open.  Instead the error message "Database does not exist or access denied" will appear.  Here's a video showing how to resolve the issue.
